Information regarding the processing of personal data by the Polish Academy of Sciences Creative Work Centre in Wierzba

1. Data Controller
   The Polish Academy of Sciences Creative Work Centre in Wierzba (Polska Akademia Nauk Dom Pracy Twórczej w Wierzbie), with its registered office at Wierzba 7, 12-220 Ruciane-Nida, and Tax Identification Number (NIP): 525-157-50-83, is the personal data controller. You may contact the Polish Academy of Sciences Creative Work Centre in Wierzba (hereinafter: "DPT in Wierzba") directly:

2. Purposes and duration of personal data processing
   Personal data will be processed for the following purposes:
   a) establishing contact via the contact form available on the DPT w Wierzbie website;
   b) marketing of DPT w Wierzbie services;
   c) sending commercial information via electronic means of communication;
   d) direct marketing using telecommunications terminal equipment.
   Personal data will be processed until the data subject withdraws their consent or objects to the processing of data for marketing purposes, or for the duration of the contact process initiated via the contact form.
3. Legal bases for the processing of personal data: Personal data are processed based on Article 6(1)(f) and (a) of the GDPR – in the event that consent is given.
4. Disclosure of personal data
   DPT in Wierzba discloses personal data in the following cases:
   1. when such an obligation arises from applicable law,
   2. when necessary to meet the needs of DPT in Wierzba:
   a) to business partners, contractors, and clients—solely regarding contact details and only if required by the nature of the concluded contract; otherwise, only upon obtaining the separate, voluntary consent of the data subject;
   b) to postal operators and courier companies.
   Furthermore, personal data may be disclosed to entities processing data on behalf of and under the instruction of DPT in Wierzba for the purpose of:
   a) providing ICT services, such as hosting or the provision and maintenance of IT systems;
   b) providing maintenance services for the reservation system.
   The processing of personal data does not involve transferring the data to third countries.
5. Rights of data subjects
   The data subject has the right to:
   1. request information from DPT w Wierzbie regarding the processing of their personal data, i.e., confirmation as to whether their personal data is being processed. If the data subject's data is being processed, they are entitled to access it, obtain a copy of it, and receive the following information: the purposes of processing; the categories of personal data; information about the recipients or categories of recipients to whom the data has been or will be disclosed; the data retention period or the criteria for determining it; the rights available to the data subject in connection with the processing of their personal data; the right to lodge a complaint with a supervisory authority; the source of the personal data if not obtained directly from the data subject; and information regarding profiling and automated decision-making (Art. 15 GDPR);
   2. to have personal data concerning her rectified. If a person learns that her personal data processed by DPT in Wierzba are incorrect, outdated, or incomplete, she has the right to request their immediate rectification or completion (Art. 16 GDPR);
   3. request the deletion of their personal data if DPT w Wierzbie no longer has a legal basis for processing them (Art. 17 GDPR);
   4. request the restriction of the processing of personal data (Art. 18 GDPR)—i.e., request that processing be stopped, with the exception of storage - in situations where:
   a) the data subject contests the accuracy of the personal data – for the period during which DPT w Wierzbie verifies their accuracy;
   b) the data subject contests the lawfulness of the processing of personal data by DPT in Wierzba;
   c) DPT in Wierzba no longer needs this data, but the data subject requires it to establish, exercise, or defend their claims;
   d) the data subject has objected to the processing – pending a decision by DPT in Wierzba regarding the validity of the objection;
   5. object to the processing of her personal data for the legitimate purposes of DPT in Wierzba;
   6. transfer their personal data—i.e., receive the personal data they provided to DPT in Wierzba in a structured, commonly used, and machine-readable format, provided the processing is based on consent—or request the transmission of such data to another controller specified by the data subject (Art. 20 GDPR).

To exercise the aforementioned rights, the data subject should send a letter or email to the contact address provided in point 1, specifying which right they wish to exercise, as well as the scope and manner of its exercise. Providing contact details in the correspondence will facilitate faster communication during the process of fulfilling the specific request—particularly if it becomes necessary to verify the applicant's identity or clarify which right is being exercised or the scope thereof (telephone contact is preferred).

6. Supervisory Authority
The supervisory authority for personal data protection in Poland is the President of the Personal Data Protection Office (PUODO), based in Warsaw at ul. Stawki 2 (telephone number: 22 531 03 00). The Contractor has the right to lodge a complaint with the PUODO regarding DPT w Wierzbie at any time—particularly if they believe that DPT w Wierzbie is processing their personal data improperly or is unjustifiably refusing to fulfill their rights.
